Program Hours:8:30Am-5:30Pm Monday through Friday.Breakfast,Lunch,Snacks provided.We provide you the caregiver with a peace of mind as you work or gain rest from the full time responsibility of caring for your love ones.We provide an organized structured program under direct supervision of a Nurse.Therapeutic,Recreational activities designed to improve,stabilize,maintain their functioning at home,and to promote health,independence through individual,group activities.Our program is designed to reduce social isolation,loneliness,stimulates interest.Each participant is evaluated,and a care plan is established to best fit his/her needs.The care plan is reevaluated,adjusted to meet the changing needs of each participant.Special care is given to meet physical,mental abilities,and special interest.Monthly team meetings to assess progress.Basic Nusing Services,Health monitoring,medication management(physician prescribed)part-time professionals consultants(Dietician,Social Worker,Occupational Therapist)on call MD.Volunteers from church/civic organizations.
